According to the official website of the State of Georgia regarding International travel into Hartsfield-Jackson Atlanta International Airport, all air passengers entering the United States from abroad are required to test negative for COVID-19. This order is effective starting January 26, 2021. If you are entering from outside the country and aren’t an authorized U.S. resident, please contact U.S. Customs and Border Protection before you travel for any additional restrictions that may apply.

STEP 1:

STEP 2:

Next you will need to agree to and sign the ID Tech Registration Disclaimer to proceed to Step 3.  This disclaimer states the following:

STEP 3:

Next you will need to enter your personal information. The new Federal Mandate for Covid Testing requires ALL patient info to be accurate.  Incorrect patient info may delay or result in the sample being rejected. During this step you would need to provide the following items:

Once you have answered the questions below the personal information section, you will have completed this section and can move along to the final step of the process.

STEP 4:

The final step is to provide our laboratory with insurance information so that we can finalize the pre-registration for the patient requiring a test.  This information includes the following:

  • Primary Insurance Name
  • Policy Number
  • Group Number
  • Insured Patient Name
  • Insured Patient D.O.B.
  • Address of Patient

Once you have completed this final section, you can click ‘I agree and Submit’ to pre-register for your test.
